Gift Card Reward Program

The Opencare Gift Card Reward Program (the “Program”) rewards registered
Opencare members (“Members”) with a gift card after they schedule and
attend an appointment with a new dentist using the
Opencare online platform. By participating,
Members agree to these terms, which are part of the Opencare
Terms of Use.
How to Earn Gift Cards
Members can earn a gift card if all the following conditions are met:

Appointment Booking: The Member must book an appointment for a
new patient exam and cleaning with a new dentist through the Opencare
online platform. Appointments booked by telephone, in person, or other
means are excluded.

Qualified Services: Only new patient exams and cleanings
qualify. Dental consultations, including x-rays, do not qualify.

Insurance Exclusions: Appointments booked using
government-funded insurance (e.g., Medicaid, Medicare, provincial
health plans in Canada, or the Canada Dental Care Plan) are excluded.

Treatment Completion: The Member must attend and complete all
standard dental treatments at a qualified dental office registered
with Opencare. If a new patient exam or cleaning is scheduled on a
different date, the Opencare Member must still attend and complete it
with the same dental office or clinic.

Age Requirement: The Member must be at least 18 years old on
the date of the appointment.

Personal Treatment: The Member must have personally received
the dental treatment.

One-Time Claim: The Program is limited to one claim per person.
Members who have previously claimed a gift card or reward within the
past two years are not eligible. Returning patients of any dental
office or clinics are not eligible

Account Consistency: The appointment must be booked using the
name on the Member’s Opencare account. If multiple appointments are
booked under one account, only one patient can claim a reward.

Claim Submission: The Member must submit their dental receipt
using the Opencare claim form. Valid
documentation includes the official dental receipt indicating the date
of the appointment, name of the dental office and dentist, treatment
rendered, and name of the patient. Treatment plans, estimates, and
appointment confirmations are not accepted.

Redemption Period: The offer must be redeemed within six months
of the original appointment date.
Claim Processing
Upon receiving the claim form and valid dental receipt, Opencare will
process the claim and provide the Member with a gift card via email in
the amount described in the original offer.
Right to Refuse Claims
Opencare reserves the right to refuse any claim if it reasonably
believes that the claim is invalid for any reason.
Privacy and Consent
By participating, Members consent to Opencare obtaining and sharing
certain information required to administer the Program. This includes
the Member’s name, address, treating dentist’s name and address, and
details of the dental treatment. This consent remains valid unless
explicitly withdrawn in writing, which also withdraws the Member’s claim
for the reward.
Severability
If any provision of these terms is held invalid, void, or unenforceable,
the remaining provisions will remain in effect.
Termination and Change
Opencare may suspend or terminate the Program or any Member’s
participation at any time for any reason. We reserve the right to
suspend Member accounts or revoke gift cards if we believe the Member is
engaging in fraudulent or abusive activity. Opencare may review all
activities related to the Program and take appropriate action at its
discretion.
The Program does not confer any legal rights to Members. All benefits
are given at Opencare’s sole discretion without contractual
consideration.
Updates to the Terms
Opencare reserves the right to update these terms at any time without
notice. Modifications will be posted on the Opencare website and are
effective upon posting. Continued participation in the Program
constitutes consent to the updated terms.
